If a concern requires immediate action, the DSL/DDSL must be informed immediately and in person prior to the concern being logged on MyConcern.
Immediate action must be taken for any concerns relating to disclosures of abuse, unexplained injuries, risk of imminent harm, concerns about abuse or harm from parents/carers, or any other person who may pose a risk to the child.
When reporting a concern staff must complete the following:
Body maps must be included for any injuries, marks or bruises, to add a body map - click body map link under the students name when raising the concern:
Once open you will be able to click on the section of body where the injury is and add a description, which must be objective and factual, information to include:
Location: Exact site of the injury on the body using the body map
Appearance: Size, approximate shape, color, and whether the skin is broken.
Swelling, bleeding, or blistering: Note any swelling, scabs, blistering, or bleeding.
You can add more than one injury to the body map, by clicking on other areas and completing the description before clicking the create body map button. This will be saved to the concern raised.
